Since you selected that you are a returning rider, we should have all your releases on file. If you aren't sure if you have signed a release within the last 12 months, please go back and select "I'm new" or let us know when you are at the barn so we can have you sign a paper release.
Health Form: In addition to this application I will be required to complete the Health Form before beginning services. I also agree to update this form annually and any time there is a health status change.
Helmets: When near or on horses, participants must wear a STM-SEI-approved riding helmet. Horses for a Change does provide helmets to those that need them. Please note that bike and ski helmets are not acceptable.
Clothing: Long pants and closed-toe shoes (with heels if possible) are required.
Lost Items: Neither Horses for a Change nor Frog Hollow Farm are responsible for lost belongings. Found items will be held for two weeks after a riding week has ended. If the rider's name is on the item, we will call to notify.
Cancelation: We require 24 notice for all lessons. Failure to do so will result in a $25 charge for each no-shoe/no-call
Bad Weather: Lessons and programs will only be canceled in the event of dangerous or threatening weather. To determine cancelations you can call the barn directly at 845-384-6424.
Siblings: If siblings are in attendance with parents and/or caregivers to the rider, parents/caregivers are responsible for the direct supervision of these children at all times. Noises and lots of activity can distract horses and other students.
Weight Limit: Rider weight limit is 200 lbs.
Right to Refuse Service: Horses for a Change has the right at any time to refuse any rider we cannot safely accommodate.

I represent that I am an adult signing on my own behalf or on behalf of my minor child, and that I wish to take riding and horsemanship instruction from Horses for a Change, Inc., at Frog Hollow Farm.
I understand that riding horses and working in the stable area has inherent dangers, and that I may be seriously injured, or even killed in a horse related accident. By way of illustration, a horse related accident includes being stepped on, kicked or otherwise struck by a hoof, bitten, or a fall from the horse.
Horses for a Change has represented to me that reasonable care will be used in the selection of horses for me to ride, and in supervising my riding lesson. I understand that Horses for a Change and its employees or agents cannot prevent accidents, and I do not expect them to do so. I have been specifically advised that I can expect a fall from a horse in the natural course of learning to ride. If at any time during a lesson I become fearful to the point that I wish to dismount, I will so advise my instructor.
I hereby specifically release, indemnify and hold Horses for a Change, Inc. and/or its employees, officers or agents harmless from liability for any claims that may arise out of my activities on the premises. I also specifically release, indemnify and hold Frog Hollow Farm and/or its employees, officers or agents harmless from liability for any claims that may arise out of my activities on the premises. In signing this contract, I am
binding myself, my survivors, or any other person or entity seeking to assert a claim on my behalf or which arises from an accident occurring at Horses for a Change at Frog Hollow Farm, even if such accident results in permanent disability, injury or death.
Performing a self-health check prior to coming and cancelling services should if I am exhibiting
symptoms of Covid-19 or have been in contact with some who has tested positive or presented
symptoms such as fever, cough, congestion.
Following Horses for a Change policies for personal protection, social distancing and disinfecting
including wearing face covering upon arrival and throughout activities.
Understanding my family members or caregivers will be required to remain in their vehicle or wait
for me in a designated area only as indicated by staff.
Horses for a Change will engage in regular cleaning and sanitizing of riding equipment, grooming supplies, helmets and frequently touched areas in between lessons as recommended by the CDC.
I agree to follow these policies and hold harmless Frog Hollow Farm, Horses for a Change, and all individuals associated with my services at Horses for a Change.
